Envision Hospice was established in 2005 by six healthcare professionals who wanted to make a difference in their community.
We are currently seeking a dynamic Licensed Practical Nurse (LPN or LVN) to provide exceptional care to patients who require one-to-one attention and monitoring in their homes in the Kitsap County, WA area, including homes, assisted living facilities, and skilled nursing facilities.
We are offering LPN opportunities that are full-time, part-time, or PRN (per diem) and have flexible schedules, depending on availability.

Essential Functions:
• Participates in the on-call rotation to provide coverage as needed.
• Client assessments, Intervention, and overall patient care, fulfilling patient care plans.
• Following physician orders and acting in compliance with the state’s Nurse Practice Act, any applicable licensure/certification requirements, and our policies and procedures.
• Strong commitment to delivering exceptional care while demonstrating compassion and integrity.
• Comprehensive and precise communication skills
• Provide one-to-one care with your patients in their homes, assisted living facilities, and long-term care.
Minimum Qualifications:
- Graduate of an accredited or state-approved nursing school
- Licensed as an LPN in the State of Washington
- Enjoy a truly patient-centered focus.
- A minimum of 1 year of experience in an acute care setting; home care or hospice experience, preferred.
