Minimum Requirements:

  • Colorado resident.
  • Bachelor’s Degree in the social or behavioral sciences from a regionally accredited college or university; OR,
  • Experience working with Long-Term Services and Supports (LTSS) populations, in a private or public social services agency, may substitute for the required education on a year for year basis.
  • Certification in a wraparound training program, OR;
  • Current enrollment in a wraparound certification program upon hire.
  • Maintain professional licensure as required by degree, when applicable.
  • Experience with home and community-based services.
  • Must possess professional and expert knowledge of communications, as well as building and maintaining relationships.
  • Must be able to travel frequently to assigned regions, including the organization’s office.
  • Strong organizational abilities including planning, delegating, program development and task facilitation.
  • Must be able and willing to work some hours outside of normal working hours to respond to emergencies or to otherwise meet the needs of the people we serve and their families.
  • Must have consistent access to reliable transportation.
  • Must possess a valid state Driver’s License.
  • Must pass a motor vehicle record (MVR), background check and other screening per state and federal requirements.
  • Successfully manage a caseload of 8-12 children/youth and their families.

Job Qualifications:

This position is responsible for implementing the wraparound process to align strategies, interventions, and supports for a child/youth and their family when a child/youth transitions to the family home from out of home placement, from a residential facility to a group home or to a foster care home, or from a group home to a foster care home, or to prevent the need for out of home placement.

  • Solid spoken and written communication skills.
  • Problem solving skills with ability to respond to crises calmly and independently.
  • Strong communication skills, both verbally and written, and knowledge of family dynamics and methods for crisis intervention.
  • Promote cooperative working relationships and positive team atmosphere, both internally and externally with providers, families, coworkers, and the people we serve.
  • Maintain good attendance as outlined in the attendance policy.
  • Passion and commitment to serve persons with developmental disabilities with dignity and acceptance of Company Person-Centered philosophy.
  • Elevated level of skill and expertise in leading personnel who support people the Company serves and/or their families.
  • Expert knowledge of family dynamics, child and youth development, and methods for crisis intervention.
  • Demonstrated understanding of trauma-informed strategies and interventions when working with children/youth and families.
  • Solid computer skills with ability to write/format letters, memos, and quality documentation.

Essential Functions and Responsibilities (included, but not limited to):

  • Maintain ongoing wraparound facilitator certification, at least every other year or as dictated by wraparound training program.
  • Develop wraparound plans that include action steps to implement strategies to address identified risk factors.
  • Coordinate among family caregivers, other family members and natural supports, service providers, professionals and case managers required to implement the wraparound plan.
  • Disseminate wraparound plans to all involved in plan implementation.
  • Assist in identifying follow-up services needed for plan implementation and maintaining stabilitization for the child/youth and their family.
  • Engage in plan monitoring and continuous revision by the wraparound team in collaboration with the child/youth and their family until the child/youth is stabilized.
  • Engage in direct visits to children/youth’s family homes for meetings, information gathering, follow-up, supports, etc.
  • Review documentation and coordinate with other professionals and team members to monitor progress.
  • Create crisis prevention and intervention plans in collaboration with the child/youth, family members, and members of the wraparound team.
  • Provide services both in person and virtually; demonstrate working knowledge of virtual platforms such as Teams, Zoom, Google Meets, etc.
  • Advocate, coordinate, and link resources and activities to assure children/youth and families receive needed services.
  • Prepare individuals and/or providers to work within the wraparound process.
  • Implement any required state-approved assessments or those required by the funding stream.
  • Establish and maintain working relationships with local community-centered boards/case management agencies, case management personnel, service providers, etc.
  • Passion and commitment to serve persons with dignity and acceptance of the Company’s Person-Centered philosophy.
  • Be clear, concise, and give all information regarding changes to appropriate parties in a timely manner.
  • Meet and follow realistic timelines/deadlines.
  • Independent in problem-solving, decision-making in well, thought out ways. Know when it is appropriate to ask for guidance or clarification. Ensure decisions are in alignment with Company mission, values, and strategic goals and all departments work together as a whole to advance the organization.
  • Lead and exemplify a positive attitude and professional appearance when representing the Company in all activities and functions of the position.
  • Knowledge of assigned children/youth’s case and all team members.
  • Maintain open communication at all times and provide excellent customer service with a person-centered approach.
  • Prioritize on-going goals and tasks.
  • Exercise discretion and confidentiality with sensitive company information following HIPAA policies and procedures.
  • Establish and ensure the execution of procedures consistent with accepted standards of state laws and regulations for youth in DHS custody and people with developmental and intellectual disabilities.
  • Knowledge of Children’s Habilitation Residential Program (CHRP) Medicaid waiver and requirements for service provision.
  • Attend team and agency meetings, as appropriate.
  • Be available to children/youth and their families during the wraparound process and communicate any needs timely and efficiently.
  • Work in partnership with all community partners; attending any meeting(s), as appropriate.
  • Perform other such duties as assigned by direct supervisor, or designee, that are within the scope of the position, as well as those reflective of professional experience, education, and ability.

Physical Demands and Working Conditions:

1. Physical Requirements - Requires repetitive manual and finger dexterity and eye- hand coordination; requires the ability to use all office equipment; and the ability to lift/carry up to 30 pounds using appropriate body mechanics.

2. Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Must be able to communicate verbally with all staff, vendors, people we serve and clients.

3. Pressure Factor – May require working under stressful conditions at times and handling emergencies. Working conditions may be noisy with fluctuating indoor temperatures. There will be pressure at times to meet scheduled appointments and deadlines and manage crisis situations. This position will interact with people we serve who are intellectually and developmentally delayed and maybe frail or confused and have the potential for verbal or physical aggression.

Job Types: Full-time, Part-time

Pay: $55,000.00 - $65,000.00 per year
