Position: Mental Health Professional
Location: EOSIS Bridge / Sauk Rapids, MN
Compensation: $62,000-$86,158 per year
Job Duties:
- Assist primary health care providers in recognizing and treating mental disorders and psychosocial problems.
- Assess the clinical status of patients referred by primary care providers through brief consultative contacts.
- Work with the primary care team to treat and manage patients with chronic emotional and/or health problems efficiently and effectively.
- Assist in the detection of at-risk patients and the development of plans to prevent further psychological or physical deterioration.
- Evaluate patient care plans with the primary care team.
- Teach patients, families, and staff about care, prevention, and treatment enhancement techniques.
- Monitor the site’s behavioral health program, identifying problems related to patient services and making recommendations for improvement.
- Attend and participate in meetings and quality improvement activities as required.
- Serve as a member of committees as requested.
- Participate in the evaluation of peers and support staff.
Job Requirements:
- Master’s degree in Behavioral Sciences or related program
- Current state licensure as a Psychiatric Nurse, Licensed Independent Clinical Social Worker (LICSW), Licensed Professional Clinical Counselor (LPCC), Licensed Psychologist (LP), Licensed Marriage and Family Therapist (LMFT)
- Licensed Professional Counselor (LPC) under supervision
- Licensed Independent Social Worker under supervision
- Must be able to pass a DHS background study or able to obtain an exemption directly from DHS

Position: Medication Aide
Location: EOSIS Bridge / Sauk Rapids, MN
Shift:
- 1st Shift (7:00 MN - 3:30 PM) EOW
- 2nd shift (3:00 PM - 11:30 PM) EOW
Pay: $19.00 - $21.38 per hour $1.00 per hour shift differential weekdays & shift differential weekends
Job Duties:
- Administers prescribed medications to patients and maintains medical records under the supervision of a nurse.
- Verifies identity of patient receiving medication and records the name of drug, and time of administration on specified forms or database.
- Presents medication to patient and observes ingestion or other application, or administers medication using specified procedures.
- May take vital signs or observes patient to detect response to specified types of medications and prepares report or notifies designated personnel of unexpected reactions.
- Documents reasons prescribed drugs are not administered.
- Records and restock medication inventories.
- Maintains patient confidence and protects the facility by keeping information confidential.
- Maintains a safe and clean working environment by complying with procedures, rules and regulations.
- Perform other duties as assigned by the Nursing Supervisor.
Job Requirements:
- High school diploma or equivalent
- Interested in being a TMA (Trained Medication Aide), we will train you!
- Must be able to pass a DHS background study or able to obtain an exemption directly from DHS
