About the available position:
EPIC - Every Person Influences Children is seeking a full-time (40 hours) Outreach and Navigator Manager. This human services position is responsible for supporting and promoting awareness of EPIC Programs while prioritizing our partnership with the regional Social Care Network (SCN). This human services position will coordinate and serve as an Assessor and Navigator connecting eligible participants to needed services, such as housing, nutrition, transportation, health care and parenting services. This position combines outreach and community engagement efforts with direct support to clients in understanding and applying for services under the 1115 Waiver, aimed at improving health outcomes and expanding Medicaid eligibility for targeted populations. Maintaining accurate records and statistics, participation in community events, and development of social media content are expectations for this position.
EPIC is looking for candidates who have the ability to:
- Conduct outreach efforts to inform communities about EPIC programs, and the 1115 Waiver Program and its benefits (program training provided).
- Organize and participate in community events, informational sessions, and presentations to increase awareness of EPIC programs and the 1115 Waiver.
- Develop and distribute materials to educate the public about EPIC programs and services, as well as the waiver program, eligibility requirements, and application process.
- Collaborate with local community organizations, health service providers, and government agencies to enhance outreach efforts and connect eligible individuals with programs, services and waiver program options.
- Represent EPIC by participating in new and existing community committees, coalitions and partner meetings throughout the region.
- Grow a network of resources for participants by connecting with other community agencies.
- Create and distribute social media content in partnership with EPIC Program Department and Director of Development.
- Post EPIC program and agency information on social media channels.
- Create EPIC program flyers for distribution.
Candidates are required to have:
- Bachelor’s degree in a related field and a minimum of two (2) years’ relevant experience in Human Services or master’s degree and one (1) year relevant experience in Human Services.
- Prior experience as a trainer or teacher is valuable.
- Case management and systems navigation experience.
- Experience connecting people in need of resources (food, shelter, transportation).
- Computer skills including proficiency in Microsoft Word, Excel, design (such as Canva) and social media.
- Database experience, Salesforce experience preferred (will train).

Essential Functions:

Participation, Eligibility and Enrollment:
- Assist clients in understanding their eligibility for services.
- Conduct assessments to evaluate clients' needs and determine program eligibility, ensuring compliance with all applicable regulations and policies.
- Provide personalized support and guidance to individuals in completing necessary paperwork for waiver applications, including assisting with the submission of required documentation.
- Serve as a navigator to help individuals through the 1115 Waiver application process, offering step-by-step assistance to reduce barriers and ensure timely access to services.
- Offer ongoing support to clients, addressing questions, concerns, and any issues that arise during the waiver enrollment or renewal process.
- Provide referrals to appropriate health and social services for clients needing additional assistance beyond the waiver program.
Compliance and Quality Assurance:
- Ensure compliance with all state and federal regulations
- Stay current with changes in policies and procedures to ensure the accuracy and completeness of client assessments and applications.
- Participate in meetings and workshops hosted by community organization and government agencies to stay informed about best practices
- Maintain up to date working knowledge of all EPIC programming and services.
Data Tracking and Reporting:
- Maintain accurate records of deliverables, client interactions, assessments, and outcomes.
- Collect and report data related to outreach efforts, program enrollment, outcomes, and client feedback to track program effectiveness and improve services.
- Contribute to program evaluations and assist with preparing reports as required by funding agencies or organizational leadership.
- Maintain reporting requirements, data entry, evaluation forms, and other contract requirements.
- Prepare billing for finance as required.
- Work with the Finance department on policies and procedures.
Other relevant duties as assigned by management.
