Job Description

Summary:
To coordinate the care for maternity patient, their family and/or caregiver across the continuum, overcoming health care barriers, providing a comprehensive assessment and facilitating timely referrals for appointments to the multidisciplinary maternity team. Serves a resource to patients, physicians and others across the continuum of maternity services.
