Duties
- Assist physical therapists in developing and implementing individualized treatment plans for patients.
- Provide direct patient care, including administering therapeutic exercises and modalities.
- Monitor patient progress and report any changes or concerns to the supervising physical therapist.
- Participate in discharge planning to ensure a smooth transition for patients moving from therapy to home or other care settings.
- Maintain accurate documentation of patient treatments and progress notes in compliance with clinic policies.
- Educate patients and their families on home exercise programs and self-care techniques.
- Collaborate with other healthcare professionals within the clinic to optimize patient outcomes.
- Stay current with best practices in physical therapy techniques, including trigger point therapy.
Requirements
- Completion of an accredited Physical Therapist Assistant program.
- Knowledge of anatomy and physiology as it relates to patient care.
- Experience or familiarity with outpatient and inpatient rehabilitation settings is preferred.
- Understanding of care plans and discharge planning processes.
- Strong interpersonal skills to effectively communicate with patients, families, and team members.

Licensed Practical Nursing( LPN)
Summary of Position: Responsible for providing professional nursing care to patients following all agency, state and federal regulations. Assists physicians and team members in coordinating patient care. Functions as primary clinical liaison between patients and physicians. Provides nursing services under the supervision of a Registered Nurse. Must communicate with RN case manager daily to ensure coordination of care is provided.
Essential Functions:
- Provides general nursing care to patients
- Observes, records, and reports patient’s condition and reaction to drugs and treatments to physicians and RN Case Manager
- Educates patient/family about diagnostic procedures, medications, nutrition, and maintenance of health and wellness
- Follow up on laboratory results, document and communicate to RN, physician and team members
- Prepare complete and accurate clinical notes. Submit all clinical documentation in time-frame per agency policy
Education/Experience:
- Current-valid Virginia Licensed Practical Nurse (LPN) license
- Associates Degree, preferred
- Current BLS for Healthcare provider certification
- Minimum of two years nursing experience, preferably in a home care agency
- 1-3 years of acute care experience
Weekly schedule:
- Monday to Friday
- Weekends as needed

PHYSICAL THERAPIST
RESPONSIBILITIES:
1. Provides physician prescribed physical therapy.
2. Improves or minimizes residual physical disabilities of the patient.
3. Returns the individual to optimum and productive level within the patient’s capabilities.
4. Participates with all other home care personnel in patient care planning.
5. Takes initial history and makes initial evaluation.
6. Performs all skilled procedures as ordered by physician.
7. Consults with physicians regarding change in treatment.
8. Writes reports to physicians regarding patient’s progress.
9. Instructs patients and family/significant others in home programs and activities of daily living.
10. Participates in Quality Assessment and Performance Improvement activities as assigned.
11. Prepares clinical and progress notes.
12. Assists physician with evaluating level of function.
13. Helps develop the plan of care and revise as necessary.
Job Types: Part-time, PRN
Pay: $30.00 - $50.00 per hour
