Primary Performance Objectives
- Coordinate the location and enrollment of benefit programs amongst individuals with limited income and resources, working primarily with older adults.
- Identify and decrease barriers for seniors to access available food benefits.
- Assist individuals in navigating the complexities of benefits applications : i.e.- Supplemental Nutritional Assistance Program (SNAP), TANF, etc.
- Partner with SC Thrive to become a benefits counselor and utilize the Thrive Hub database to submit applications on behalf of clients.
- Meet with clients in the office, in the community or remotely as needed to assist with applications.
- Make appropriate referrals to food and nutrition programs and assist clients in utilizing other community resources.
- Conduct community wide outreach with individuals, churches, and other groups on SNAP program eligibility.
- Develop partnerships to increase SNAP awareness and application submission.
- Ensure that enrollment and outreach activities are tracked and recorded for data reporting purposes.
- Attend community events, as scheduled.
- Advocate alongside families for access to services to achieve their goals .
Job Task Summary
Duties and Responsibilities
- Become familiar with SNAP policies / state regulations, procedures, and keep abreast of changes.
- Raise community awareness of SNAP and participation requirements of the program.
- Develop rapport with target population and answer any SNAP related questions.
- Screen and advise clients on documentation and eligibility requirements for SNAP.
- Provide closed loop referrals to individuals seeking assistance.
- Follow up with families and agencies, identify barriers and provide solutions.
- Maintain strong understanding of available community resources
- Maintain positive and professional relationships with community partners
- Assess families using the Foster Kinship assessment tools.
- Assist with support groups, events, and training classes as necessary.
- Serve all individuals and families in an equitable, culturally responsive and empowering manner.
- Collaborate with Epworth staff to enhance the mission and ministry as a whole.
- Attend individual and group supervision meetings and trainings as scheduled.
- Adhere to the general principles, ethics and qualifications for employment as outlined in the staff manual.
- Follow direction and guidance of immediate supervisor. Additional duties may be assigned as needed.
- Participate as appropriate in the PQI process as assigned.
- Driving may be required - Epworth will provide a mileage rate for reimbursement.
- Understand and follow personnel policies and practices as outlined in the ECH Staff Manual and / or department manuals and in compliance with the SCDSS Foster Kinship Model, as well as those outlined in the Contract with the South Carolina Department of Social Services, and the Caring for our Own model.
Qualifications
- BSW or BA / BS in related human services field is preferred but candidates with an associate's degree plus an extra two (2) years' experience in a related field or candidates with a High School diploma with a minimum of four (4) years' extra experience in related field.
- At least two (2) years' experience with families and knowledge about community resources and services. Experience in advocacy work on behalf of children and families preferred.
- A belief system and faith practices that are compatible with those of the United Methodist Church.
