The Population Health Navigators at Erie connect patients with a range of services to address the social drivers of health. This includes linking patients with internal resources at Erie like behavioral health or case management services as well as connecting patients to local community resources such as rent / utility assistance, food programs, transportation, and legal aid. The Population Health Navigator works closely with patients to understand their unique needs and curate individualized resources to overcome barriers. - Compensation is based on each candidate's experience, skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered. Main Duties & ResponsibilitiesScreen Clients for needs around social drivers of health and connect to internal and external resources.Link to and / or enroll patients in services, benefits, and programs for which they are eligible (pharmaceutical programs, LINK, financial applications for hospital referrals, etc.).Assist patients in digital navigation, including MyChart enrollment, setting up telehealth capabilities, and other digital assistance related to health.Conduct health outreach activities, through a variety of methods including campaign texting, appointment recalls, 1on1 patient phone calls, bulk referral ordering, and MyChart, as well as others.Assist with distribution and tracking of SDOH resources, including bulk mailing SDOH resources.
