Erie Family Health Centers, a nationally recognized top workplace with 13 sites in Chicago and suburbs, is looking for a valuable addition to our Behavioral Health team! The Behavioral Health Consultant (BHC) position is a behavioral health provider who operates both in a consultative role within the primary care setting and serves as a member of the Behavioral Health Program. The employee provides PCP-initiated consultation services that may include differential diagnosis, screenings, psychoeducation, brief intervention, and referral for further treatment. In addition, the BHC will maintain a case load, providing individualized mental health assessments, treatment, and counseling, as appropriate.

Main Duties & Responsibilities
- Screen Clients for needs around social drivers of health and connect to internal and external resources.
- Link to and/or enroll patients in services, benefits, and programs for which they are eligible (pharmaceutical programs, LINK, financial applications for hospital referrals, etc.).
- Assist patients in digital navigation, including MyChart enrollment, setting up telehealth capabilities, and other digital assistance related to health.
- Conduct health outreach activities, through a variety of methods including campaign texting, appointment recalls, 1on1 patient phone calls, bulk referral ordering, and MyChart, as well as others.
- Assist with distribution and tracking of SDOH resources, including bulk mailing SDOH resources. This also includes Top Box Foods distribution, ordering Uber Health rides, and distributing other material resources.
- Use Epic to document work, create panels and outreach lists, respond to referrals, and communicate with clinical care teams.
- Build effective relationships with local social service agencies and hospital partners, advocating for clients in the process.
Education
- High School diploma or equivalent required
- Associates degree, some college or bachelor’s degree a plus
Skills and Knowledge
- Bilingual in fluency (English/Spanish) a plus (May be required to assist or work with non-English/Spanish only speaking patients and/or guests.)
- Verbal and written communications skills, including high level of comfort presenting to groups.
- A minimum of two years’ experience in community health, social or community services, or comparable experience preferred.
- Intermediate proficiency in Microsoft Office Preferred
- One (1) year of outreach, education, community health, or customer service experience
