Erie Family Health Centers, a top workplace in Chicago and suburbs, is looking for a valuable addition to our Lending Hands for Life team ! The Patient Navigator builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy. Using methods such as internal and external referrals and agency data, the Patient Navigator will work to identify individuals who are HIV , HepC , or vulnerable to HIV but are not engaged in medical care. The Patient Navigator will work to reengage patients living with HIV and / or HepC and those patients vulnerable to HIV and HepC in medical services (by phone outreach, text message, letters, home visits, and other methods).
