The program coordinator is responsible for developing effective programs, overseeing activities, and measuring overall success. Their duties include implementation, support and coordination of a program, managing administrative tasks, tracking the program budget, and ensuring that the program meets guidelines and requirements. Program coordinators develop new initiatives and work with teams to ensure smooth operations.
Job Duties Include:
This position is involved in a team-based approach to care. Team members are trained to meet the highest level of function for their role as per the State of Tennessee/Georgia guidelines. Is trained and assigned (as per their role and responsibilities) to coordinate care for patients and is trained and assigned to support patients/families/caregivers in self-management, self-efficacy and behavior change. Is trained and assigned (as per their role and responsibilities) to manage the practices� patient population. Participates in the practice's quality improvement process and performance evaluation.

- Planning and coordination of a program and its activities
- Develop process improvement and education towards best practice
- Ensuring implementation of policies and practices
- Maintain budget and track expenditures related to program
- Help build positive relations within the team and program parties
- Schedule and organize meetings/events and maintain agenda
- Ensure technology is used correctly for all operations
- Prepare paperwork for annual certifications or program requirements
- Keep updated records and create reports or proposals
- Support growth and program development

Required:
- Bachelor's degree in business management, administration, education, process improvement or health related field, or currently working towards a bachelor's degree.
Experience:
Required:
- Experience in health care related program/project management.
Preferred:
- Experience in quality improvement, process improvement models (Six Sigma, ISO9000 or FOCUS-PDSA)
- Experience in data collection and analysis techniques (quantitative and qualitative)
- Knowledge of NCQA PCMH programs
