Job Summary:
Educate and assist families and patients about Community Access Program (CAP) including appropriate use of services in order to foster access and reduce inappropriate use of the emergency and hospital services and prevent nursing home placement. As required, provide assistance with eligibility determination for behavior health, substance abuse and social programs. Will assist in identifying funding sources in order to qualify patient for community assistance from a variety of special funds and agencies. Coordinates and facilitates navigation and continuity of care via Community Access Program (CAP)/ primary medical home in conjunction with development of a discharge plan of care from area hospitals and providers.
This position is involved in a team-based approach to care. Team members are trained to meet the highest level of function for their role as per the State of North Carolina Community Access Program. Is trained and assigned (as per their role and responsibilities) to coordinate care for patients and is trained and assigned to support patients/families/caregivers in self-management, self-efficacy and behavior change. Is trained and assigned (as per their role and responsibilities) to manage the practices� patient population of the Community Access Program. Participates in the practice's quality improvement process and performance evaluation.
Education:
Required:
Bachelor's Degree in Human Services or Similar.
Experience:
Required: Self-starter with three (2) years� experience as a Case Manager or similar with demonstrated skills working with multidisciplinary clinical team; communication skills and computer literacy is a must; hospital, outreach or outpatient experience is required as is knowledge of community resources.
Preferred:
Additional experience w/ commercial, Medicare or Medicaid providers.
Position Requirement(s): License/Certification/Registration
Must maintain full, unrestricted Driver's License. Must have automobile for travel and maintain current auto liability insurance. BLS must be completed prior to going to unit/department and maintained going forward.
Job Type: Full-time
Pay: $14.51 - $23.22 per hour
Expected hours: 37.5 per week
