POSITION SUMMARY : The Population Health Administrator supports the Population Health Department in managing logistics and facilitating clinic-wide Quality Assurance and Population Health efforts. The Population Health Administrator provides supervisory support to team members under the leadership of the department director, in addition to coordinating logistics of population health events aimed to increase access to care for our most vulnerable patients, thereby reducing health disparities. Additionally, the Population Health Administrator will serve to monitor the functions of the Department and ensure progress toward goals in collaboration with the rest of the team.
ESSENTIAL FUNCTIONS
- Provide supervision to Population Health Department staff members in collaboration with the AMD for QA / PH / CHW.
- Monitor process metrics of the Population Health department projects, identifying areas of challenge and success in collaboration with Population Health RN and AMD for QA / PH / CHW.
- Work with the AMD for QA / PH / CHW and Development team on QA and Population Health grants.
- Work with the AMD for QA / PH / CHW and Medical Administration team to ensure that Population Health Department-related grant requirements are met.
- Support the department with workflows for pulling patient lists from the population health software.
- Interface with other clinic staff to coordinate patient outreach projects, including using CareMessage and managing appropriate follow-up.
- Coordinating Population Health event logistics, including mammogram van events and other future partnerships.
- This includes, among other things, coordination between EHC and partner organizations, facilitating event details, and ensuring proper follow-up on results.
- This may include the exploration of new opportunities and partnerships to increase the accessibility of care to our patients.
- Manage administrative tasks for the department’s staff, including timesheets, schedules, and other related duties.
- Coordinating Quality assurance-related activities, including QA team meeting schedules, peer chart reviews, hub QA meetings, and other required clinic activities.
- Assist with other population health and quality assurance needs as directed by the AMD for QA / PH / CHW.
