What is PACE?

PACE, or Program of All-Inclusive Care for the Elderly, is an alternative care model for those over 55 years of age requiring nursing home level of care which aims to keep participants living at home in the community. PACE is a collaborative, interdisciplinary approach which coordinates all levels of medical and social needs, including prevention, wellness, socialization, primary care, and access to specialists and other services.

The Medical Director of PACE is responsible for providing clinical leadership for the care and treatment of participants served by Esperanza’s PACE. The Medical Director provides primary care to participants as well as clinical direction and guidance to the PACE interdisciplinary team (IDT) to ensure quality care.

Primary Duties and Responsibilities :

  • Works cooperatively with the Vice President of PACE to ensure quality care delivery to all participants, affirm the PACE mission, and educate team members on the PACE service delivery model.
  • Coordinates the work of medical staff, chair medical staff meetings, and supervise educational plans of fellows, residents and students who rotate at the PACE center.
  • Recruits and hire contract providers as appropriate for the program.
  • Works cooperatively with clinical staff, including integrating primary care providers into the PACE team, as needed.
  • Serves on appropriate organizational and corporate-wide committees.
  • Leads specific projects as appropriate and directed by the Chief Medical Officer and Vice President of PACE.
  • Serves as a leader in the presentation of programs and research at regional, state, and national meetings and represents PACE at appropriate local, state, and national forums.
  • Assists in developing clinical practice standards for PACE physicians, APRNs, and nursing staff.
  • Oversees PACE Quality Improvement (QI), working collaboratively with the QI Coordinator to identify, monitor and track quality improvement initiatives and grievances.
  • Participates in organized and structured reviews of adverse outcomes and lead towards a culture of safety.
  • Assists in the development of written guidelines and protocols as appropriate for the PACE program.
  • Supports the PACE organization in fulfilling regulatory responsibilities when enrolling all eligible applicants.
  • Assists in development and review of PACE policies and procedures.
  • Works with the Vice President of PACE to monitor and educate the IDT on total cost of care, cost-effective prescribing habits, medical appropriateness, and utilization reports and management.
  • Ensures regulatory compliance regarding medication management, including proper dispensing, administration, and avoidance of fraud, waste, and abuse.
  • Participates in medical reviews as needed and directed by the Vice President of PACE.
  • Maintains a regular schedule of medical participation in all components of the program and maintain medical staff coverage during PACE center business hours.
  • Establishes and maintains a reliable and responsive care network to supplement primary care and assist in the care of PACE participants – including consultants, hospitals, nursing facilities, and a system for off-hours coverage.
  • Performs all functions and duties as required as a member of the PACE Interdisciplinary Team (IDT), including, but not limited to, participation in IDT meetings, family conferences, grievances, service determination requests, CMS audits.
  • Coordinates with IDT to assist primary care providers, as needed, in developing individualized plans of care for each PACE participant.

Requirements :

  • Graduate of an accredited medical school.
  • Board certification in primary care (internal medicine, family practice) required.
  • Fellowship training in geriatrics with board certification preferred.
  • 3 years of experience as a primary care physician.
  • 2 years of experience working with older adults strongly preferred.
  • Evidence of program or clinical leadership, minimum of one year.
  • Licensed as a physician in the State of Illinois; specialty / board certification in geriatric care highly preferred.
  • Controlled substance license.
  • Bilingual (English and Spanish) required.
  • Must be certified in CPR / BLS (Basic Life Support) by the American Heart Association.
  • Must possess valid driver’s license and documentation of current automobile insurance at agency-specified levels.
  • Must comply with requirements of other facilities (hospitals, LTC (Long Term Care) facilities, etc.) related to flu or other vaccines if job duties require providing services in those facilities.
