What are the responsibilities and job description for the Remote Health Monitor position at Geriatric Medical Associates?
Geriatric practice looking for Medical Assistants with exceptional organizational skills. Must be a people person with a professional demeanor and the ability to work independently and be able to multitask.
Duties: The position is a Remote Patient Monitoring or Chronic Care Management program, doing health and wellness telephone calls with them on a monthly basis. The ideal candidate is comfortable providing telephonic high-quality Chronic Care Management (non-face-to-face) services as an extension of the clinical staff of physician practices managing patients with 2 or more diagnosed chronic conditions.
Care Coordinators call each of their patients to cover the following:
- Provide initial risk assessment gathering the clinical elements necessary to determine a patient specific Care Plan
- Chart review Med Reconciliation
- Prescription refill requests
- Referral coordination
- Prescription refill requests
- Other activities designed to close gaps in care
- Work with patient to plan and monitor their care as determined by the PCP.
- Promote compliance with PCP/Specialist office visits.
- Act as a patient advocate, liaison and information resource.
- Provide chronic care education for chronic/complex conditions as determined.
- Provide appropriate health education.
- Ability to maintain effective and organized system to ensure timely patient flow.
- Spanish Speaking (Preferable)

Pay: $18.50 - $19.50 per hour
