Nursing Care Coordinator
We are seeking a compassionate and skilled Nursing Care Coordinator to join our team at Essentia Health Sandstone.
Job Summary:
As a Nursing Care Coordinator, you will be responsible for organizing and providing nursing care to patients through assessment, planning, intervention, and evaluation. You will work collaboratively with other healthcare professionals to ensure high-quality patient care and meet the mission and goals of the facility and Essentia Health.
Duties and Responsibilities:
- Assess patient needs and develop individualized care plans
- Delegate tasks to other nursing personnel based on licensure, preparation, and job descriptions
- Respond to emergency calls and assist ambulance teams as needed
- Maintain accurate and up-to-date patient records
Requirements:
- BSN or ADN degree from an accredited school or college of nursing
- Current license with the appropriate State Board of Nursing
- Trauma Care After Resuscitation (TCAR) or Trauma Nursing Core Course (TNCC) certification within 1 year of hire
