Job Description
As a Care Coordinator with the Minnesota Senior Health Options (MSHO) program, you will play a vital role in assisting members who are enrolled in dual eligible Minnesota Health Care Programs.
Your responsibilities will include conducting assessments, coordinating services, and helping members achieve their health goals. This may involve making in-person visits to enrollees' homes.
Licensure/Certification Requirements
- Certification/Licensure Requirement: Current RN MN License
- Must have reliable transportation and a valid driver's license
