Essential Therapy Solutions started in 2007 in response to the overwhelming number of families in our Arizona community requiring services. We obtained school contracts as well as becoming a Qualified Vendor with the Arizona Division of Developmental Disabilities (DDD). We also contracted with other agencies holding Arizona Early Intervention Program (AzEIP) contracts to deliver services to the birth to 3 population.

Service Coordinator Job Description
Essential Duties:
- Process referrals, schedule initial meetings with families, conduct screenings using standardized protocol and tools approved by AzEIP, and ensure timely delivery of services
- Assess nature of clients' situations by interviewing and reviewing personal history
- Ensure proper maintenance of child files, required documentation and the AZ DES automated system
- Coordinate, attend and schedule assessments; IFSP (6 months and annual), IEP, eligibility determination meetings, transition planning; notify all interested parties of meeting dates and times
- Communicate client updates to all relevant parties
- Facilitate referrals to other healthcare professionals and programs
- Demonstrate knowledge in AzEIP Policies and Professionalism, Child Development, Family Professional Partnerships, Teaming, Assessments and Evaluation, Planning, and Intervention
- Demonstrate knowledge in AZ DDD/DES policies
Qualifications:
- A Bachelor's Degree in Early Childhood, Child Development, Nursing, Psychology, Social Work, Special Education, or a Bachelor’s degree in a closely related field (e.g., Psychology, Social Work, Elementary Education, or Sociology) with review of coursework and approval by AzDES/AzEIP.
- Candidate must successfully complete a background clearance and obtain Level One Fingerprint Card Clearance
- Travel to attend meetings, training and appointments
- Excellent written and verbal communication skills
- Bilingual English/ Spanish for this position is a plus
