Responsibilities
- Assess client needs and develop personalized service plans
- Connect clients with community resources and support services
- Monitor client progress and adjust service plans as needed
Requirements
- Bachelor's degree from a regionally accredited or New York State Registered college or university
- Or an Associate's degree and 3 years of experience providing Human Services to the public
- Possession of a valid New York State Driver's license at the time of appointment

Special Requirements
This title has been approved without the need to take an exam ending June 30th 2025, as part of the HELPS Program through the NYS Department of Civil Service. Candidates appointed permanently to this title will NOT need to take the Civil Service Exam and will remain in their position even when the HELP Program ends.
