We are looking for a highly motivated individual to join our team as a Targeted Case Manager. This individual should possess compassion for those who suffer from addiction and have a non-biased view on recovery treatment options, including medication assisted treatment.
Job Description
The Targeted Case Manager is responsible for care coordination during the primary and structured recovery process while under Ethan Health care. Targeted Case Management provides client advocacy while linking and referring clients to community resources to promote independence, growth and support that will enable individuals to maintain long-term sobriety. Responsibilities
- Collect information about clients through interviews, observation, and assessments
- Advocates for and assists clients with gaining access to necessary medical, social, educational services to meet specified needs and accomplish defined goals
- Act as patient advocate to coordinate required services or to resolve emergency problems in crisis situations
- Facilitate communication between community resources, referral sources, and others involved with client for continuity of care.
- Continuously check that services are being provided in accordance with the care plan.
- Utilize the SAMHSA TIPS for Comprehensive Case Management for Substance Abuse Treatment at a reference to providing care
- Complete state and agency required initial and ongoing training
- Performs other duties as assigned by management
Qualifications
- Minimum 1 year of documented sobriety for individuals in recovery
- Knowledge/experience in the Twelve Step Recovery Program
- A bachelor's degree in Psychology, Sociology, Social Work, Family Studies, Human Services, Counseling, or Nursing with 5 years of full-time employment experience working with individuals with SMI, SED, or SUD and co-occurring chronic or complex physical health condition.
- Master's degree in Psychology, Social Work, Family Studies, Human Services, or Clinical Counseling, and two years of full-time experience providing service coordination or linking/referring for community-based services for individuals with SMI, SED, or SUD and co-occurring physical or behavioral health disorders.
- Computer literate and able to communicate clearly, both in writing and verbally with clients, administration, the treatment team, and family members
- Must pass a background check and drug screen
- Must be excluded from the Federal Exclusion Database

Job Type: Full-time
Pay: $18.00 - $20.00 per hour
