Overview
Ever Well Health Systems is seeking a Residential Solutions Manager to join our Residential Solutions Team. The ideal applicant is an organized, compassionate, and flexible team player! Through communication and collaboration with Case Managers, Conservators, and other placing agency staff, the Residential Solutions Manager leads team members in the evaluation and placement of residents at Ever Well’s licensed facilities. The incumbent ensures the Residential Solutions Team follows established workflows from initial placement referral through the resident’s orientation period. This includes ensuring relevant data and documents have been received timely and have been accurately entered into our electronic databases. The Residential Solutions Manager manages and ensures timely responsive evaluation, assessment and orientation of potential residents throughout the admissions process. This role supports strong customer relationships with internal and external customers delivering exceptional customer service.
Duties
- Participates in the recruiting of new residential solutions staff and ensures proper training, coaching and management of assigned staff to ensure systems and processes are followed.
- Ensures the organization’s daily census goals are met, while always placing individual residents in the Ever Well facility/program that best meets their treatment needs.
- Answers admissions related questions related to the acceptance and retention requirements and procedures, while maintaining excellent front-of-house relationships with customers, residents, and service lines.
- Ensures staff acquire initial referral documentation and the required data has been entered into database(s) to determine appropriate admission and/or follow-up plan.
- Coordinates with service line Directors to ensure all referral and admissions processes and procedures from initial referral stage to resident admit are followed to achieve organizational mission and objectives.
- Participates in the promotion of Ever Well’s programs and makes frequent contact (at least monthly) with county customers to increase referrals by making personal contact and attending specific county meetings/relevant conferences.
- Maintains CCR Title XXII regulations and follows agency requirements, including initial and ongoing resident documentation standards for all admissions across Ever Well’s facilities.
- Supervises assigned staff to ensure dignified admission of accepted referrals including room assignments, transportation, special considerations/conditions for all new residents.
- Provides and prepares data for periodic reports to produce Key Performance Indicators (KPIs).
- Participates in agency meetings and training as assigned or required. This includes obtaining an ARF and RCFE Administrator’s certification (within 90 days of hire) and keeping certifications current.
- Performs other duties as assigned.
Compensation
This job is comprised of a base salary, and commission structure based on established key performance indicators of success.
Schedule
This is an exempt role, with a typical schedule of Monday-Friday.
