The Case Aide acts as a member of the interdisciplinary team, designated to support case management functions of the program. The Case Aide is responsible to ensure daily case management services are provided to youth in a timely manner. Through the support they provide to the case management and Clinical Department the Case Aide ensures comprehensive documentation of family unification efforts and the safe and timely release of youth from the program.

What does this job do specifically?

- Coordinates the timely collection and completion of required documentation for youth in care.
- Provides daily, weekly and monthly submission of required updates to internal and external team members.
- Provide support to ensure youth receive required telephonic contacts with family of origin and identified sponsor. This may include supporting the provision of supervised phone calls or extra phone calls as clinically indicated.
- Provide safe transportation for youth utilizing agency vehicles. Transportation could include driving to appointments, court, attorney visits, consulate interviews, and / or providing transportation to the airport and assisting with the facilitation of youth release for unification purposes.
- Assist with filing, closing and overall daily maintenance of client records in Everstand’s electronic health record (Cx360) and the UC Portal. Ensure assigned case records meet documentation standards as defined by both federal and state regulations.
- Assist with identifying resources available to support youth and sponsor post release. Assists case management with the development of discharge safety plans reflective of the identified youth needs and corresponding resources.
- Assist case management with creating discharge packets with discharge documentation and provide exit orientation to client and discharging client from UC Portal as part of discharge process from the program.
- Assist with coordination of travel arrangements for staff and youth as it relates to youth release from the program and unification with sponsor. This may include booking airfare, hotel and rental car.
- Ensure the provision of follow up service phones calls conducted post release of the youth from the program. This includes but is not limited to responsibility for completed the 30 day Safety Well Being calls.
What experience and education do I need?
Required Education and Experience
- A high school degree and 3 years’ experience working in a social service environment.
- Fluent in both English and Spanish
- At least 1 year of experience in a professional office environment.
Preferred Education and Experience
- Child Welfare and/or case management experience.
- Experience working with Unaccompanied Children, refugee or recently migrated population.
- Associate degree
