The Therapist / Case Manager acts in a dual role, supporting both the case management and therapeutic services for all assigned youth. The Therapist / Case Manager will be the primary point of contact for family members and deliver clinical intervention outsides of educational hours. Interventions provided include but are not limited to individual, group and family therapies. All Everstand therapists are credentialed and certified trauma professionals within their first year at Everstand. The Therapist / Case Manager works closely with all members of the care team to design and implement guidance and treatment plans for the youth in order to develop, support and encourage safe behaviors, self-regulation and healthy relationships.

Make it happen."Take the lead in being the primary contact for youth's identified supports.Maintain effective working relationships with families, children / youth, colleagues, consultants and community agencies to include scheduling home visits, notifications of serious incidents, follow-up on all ongoing documentation needs.Provide clinical services that are trauma-informed and aligned with Everstand's purpose and values.Provide individual, group, and family therapy, within the scope of professional license, to children, adolescents, and their families.Learn and implement evidence-based practices selected by the organization, which may include attending on- or off-site training, participating in ongoing supervision, consultation, and / or continuing education, committing to delivery of the practice as expected by Everstand, following practice guidelines with fidelity, and engaging in fidelity monitoring.Locate and facilitate referrals to community-based resources within the youth's home community. Coordinate with the Everstand staff members and community resources to address clients' service needs. Ensure that the service needs are met.Facilitate family focused skill building or engagement groups monthly, ensuring that there is proper documentation of these groups.Ensure that each youth is receiving the following :Ensure youth receive an average of 8 face-to-face clinical service hours weekly (Individual Case Management, Individual Therapy, Family Therapy, Group Therapy, Life Skills Group) outside of school hours.
At least 1 contact per week with youth's external care team and family (in person, virtual, email or phone).IEP service frequency and duration are being met weeklyProvide effective management of assigned cases which includes but is not limited to : developing, monitoring, and reviewing Individual Treatment Plans, Safety Plans, and Transition / Discharge Plans, and collaborating in transition / discharge planning.Serve on the Interdisciplinary Care Team for each youth served, to include consultation with the providing psychiatrist regarding client needs and interventions, and working collaboratively with Assistant Program Director, Unit Supervisor, Certified Residential Child and Youth Care Professionals (RCYCP), Lead Treatment Support Specialist, Educational Coordinator, Special Educator, RN, Pediatric NP and Psychiatrist under the guidance of the Assistant Program Director.Evaluate the child's / youth's / family's progress towards goal attainment on a designated basis in line with individual treatment and transition / discharge plan goals that are individualized, have strengths and service needs clearly identified, and are developed in behaviorally measurable terms.Complete psychosocial intake assessments and ongoing clinical assessments that include participation by child / youth and parents / guardians.What experience and education do I need?
Required Education and Experience
Master's Degree in Social Work, Counseling, or a related field from an accredited college or UniversityCurrent LGSW, LGPC, LCSW-C, LCPC, LGMFT, LCMFT, LGPAT, or LCPAT Maryland State LicensePreferred Education and Experience
