The Wraparound Facilitator is the member of the care team that will engage high-need youth and their families in developing, implementing, and monitoring an intensive, individualized care plan. This includes helping the care team develop a natural support system, working on the development of an integrated plan and establishing a strong relationship with the family. This partnership is important as the youth and family work to address challenges and make changes to their lives. The Wraparound Facilitator helps the family develop a positive view of the future through various activities, identifying and promoting resources available for the youth and family in their home community while safeguarding the process to ensure that the principles and steps of the Wraparound process are delivered with the highest possible fidelity to the model.

What does this job do specifically?

- Coordinate initial meetings with youth and family, ensuring the completion of intake documentation and necessary assessment tools in the identification of strengths, service needs, and transition plan.
- Facilitate the development of the Wraparound care plan for each youth and family ensuring that the plan demonstrates youth and family voice and includes clear needs statements, strength-based strategies, and outcomes that are measurable and specific.
- Collaborate with Wraparound care team to develop a crisis and safety plan to ensure the safety of the youth, family, and community.
- Coordinate and facilitate ongoing child and family team meetings.
- Meet agency productivity and documentation expectations with regards to weekly meaningful contact with youth and families.
What experience and education do I need?
Minimum Required Qualifications:
- Bachelors of Arts (BA) or Bachelors of Science (BS) in a human service or related field from an accredited college or university
Preferred Education and Experience
- Master’s Degree in a human service related field from an accredited college or university, with professional licensure.
- Documented experience, training or credentialing in Wraparound programming.
- Experience working with or living with youth that have complex emotional needs.
- A solid knowledge of community resources and different child-serving systems.
