Case Manager - Transitional Living Program

Receives and screens all telephone and e-mail inquiries for Steppingstone services. Assesses via standardized admission criteria what services a referral source or potential client may require. Assists with eligibility determination process and investigates service availability by reviewing the agency census report database. Must be knowledgeable both of the agency services and community service delivery systems in order to make timely and appropriate referrals. Provide case management services to youth ages 16-21 who are in foster care, homeless, or at-risk of becoming homeless.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following (other duties may be assigned):

Intake Coordination:

  • Field all intake inquiries from referral sources.
  • Make referrals to outside agencies when ECH cannot satisfy request. Document all relevant information regarding inquiries into agency database.
  • Conduct interviews with potential clients for Steppingstone both in-office and in the community. Make eligibility determination regarding funding and space within the program.
  • Demonstrate ability to be courteous, empathetic, and tactful with referral source and potential clients. Keep confidential all information collected and shared regarding clients and families.
  • Write and send acceptance or rejection for service correspondence to referral source and clients.
  • Compile monthly reports and annual reports data related to inquiries and referral for service information. Schedule and conduct placements; completing intake paperwork with clients and/or legal guardians.
  • Attend weekly referral meeting with Program Director to make eligibility determination regarding funding and space within the program.
  • Facilitate the on boarding process and admissions to the program.

Case Management:

  • Attend Steppingstone team meetings.
  • Collaborate with clients in developing individualized service plans and collect outcome measures. Attend and participate in judicial hearings, school meetings, PPRTs, and SPRs.
  • Provide service coordination in the community (i.e. landlords, public assistance, and other service providers). Teach daily life skills to youth within the program.
  • Support youth with mental health and medical services. Provide assistance in the event of a crisis or emergency.
  • Assist youth with developing educational and vocational plans.
  • Provide on -call support one weekend per month and one holiday per year.
  • Other duties as assigned.

Education and/or Experience: Must be at least 21 years of age and have a bachelor's degree from a college or university in a field related to human services and have 2 years of experience working with people. Must be of good moral character and able to serve as a positive role model for youth; sometimes work under stressful situations and seek on-the-job training.
