Case Worker

Position Summary
The Case Worker is responsible for providing case management, advocacy, crisis intervention, and referrals to low-income individuals and families seeking assistance through the Social Services Department. This role involves direct client support, community engagement, and collaboration with service providers to enhance client well-being, stability, and violence prevention efforts.
Qualifications
- Education & Experience: BA/BS in Social Work, Psychology, or a related field, plus two years of relevant experience.
- Skills & Abilities:
- Strong oral and written communication skills.
- Experience in counseling, advocacy, and crisis intervention.
- Ability to work effectively with diverse populations.
- Strong judgment and ability to handle high-stress situations.
- Additional Requirements:
- Reliable transportation, valid driver’s license, and proof of insurance.
- Local travel as needed.
Key Responsibilities
Client Support & Case Management
- Conduct individual client assessments to determine needs and available resources.
- Provide crisis intervention and emotional support to clients in distress.
- Develop and implement individualized care plans for client stability and progress.
- Assist clients with applying for local, state, and federal benefits.
- Facilitate transportation arrangements for essential services, court hearings, medical appointments, and employment.
- Conduct follow-ups to ensure continuity of care and assess client progress.
- Maintain accurate and confidential client records, case notes, and reports.
Advocacy & Community Collaboration
- Advocate for clients to secure benefits, housing, healthcare, and legal assistance.
- Work collaboratively with social service agencies, law enforcement, hospitals, and community organizations to strengthen response efforts.
- Provide referrals to appropriate agencies for additional services.
- Engage with schools, community centers, and faith-based organizations for outreach and awareness programs.
- Align services with Office of Violence Prevention initiatives to support community-centered strategies.
Education, Training & Prevention
- Facilitate educational workshops on violence prevention, trauma recovery, life skills, and conflict resolution.
- Provide training and awareness sessions for families and community members.
- Offer consultation and training to community partners on violence response and intervention strategies.
Data Management & Reporting
- Maintain accurate case records and documentation in compliance with program guidelines.
- Track and report case progress, service utilization, and client outcomes.
- Document community engagement activities, workshops conducted, and collaborative efforts.
- Participate in team meetings, training, and professional development opportunities.
Program Support & Administration
- Support seasonal programs, including holiday assistance, parking lot Fridays, back-to-school drives, and emergency relief efforts.
- Stay informed about evolving community resources, services, and policy changes that impact client care.
- Assist with agency operations, fundraising, outreach, and community awareness activities.
- Represent the agency at community events, meetings, and trainings as requested.
