What are the responsibilities and job description for the RN Care Coordinator position at EveryAge?
This position is accountable for clinical management functions to assure delivery of quality health care and services consistent with regulatory compliance and facility policies, procedures, standards and financial objectives. The RN Care Coordinator’s essential duty is to provide direct resident care. The RN Care Coordinator oversees all nursing personnel on the assigned shift.
Minimal Education:
Graduate of accredited school of nursing
Bachelor’s degree in nursing or related field preferred.
Minimal Experience:
Minimum of three (3) years Registered Nurse experience in a skilled nursing facility.
One (1) year of supervisory experience preferred.
Other Qualifications:
Current Nursing License
CPR certification
Strong communication, leadership skills and interpersonal relationship skills.
Demonstrated clinical competency and provision of expert care to patients with either predictable or unpredictable
