Responsibilities:
• Plan and coordinate hospice care, utilizing hospice experience to develop individualized care plans tailored to patients' physical, psychological, and social needs.
• Lead and supervise hospice aides, ensuring they deliver compassionate and consistent patient care that aligns with hospice standards.
• Conduct patient assessments and adjust care plans in collaboration with the Clinical Manager to address evolving patient needs effectively.
• Educate patients and families on end-of-life care options, providing compassionate guidance and answering questions based on hospice expertise.
• Collaborate with interdisciplinary team members, including physicians, social workers, and hospice aides, to provide holistic, high-quality care.
Qualifications
• Graduate of an accredited school of nursing; current RN licensure in the state.
• Minimum of 1-2 years of recent acute care experience in a clinical setting, with 1-2 years of experience in home health or hospice care preferred.
• Basic Life Support (BLS) Hands-On CPR certification.
• Strong observational, communication, and problem-solving skills.
• Valid driver’s license, active auto insurance, and a reliable vehicle, as travel is required for home visits.
