Social Worker Responsibilities You'll Love:
- Provide counseling and emotional support to residents and their families
- Assess social and psychological needs of residents and develop care plans
- Advocate for residents and ensure they receive necessary services and resources
- Assist with discharge planning and community referrals
- Coordinate with interdisciplinary team members to provide holistic care
- Maintain accurate and confidential documentation as required

- Active Social Worker license in Delaware
