Key Responsibilities:
- Evaluate and Enhance Care: Oversee the quality of care for long-term residents, ensuring their needs are met with excellence.
- Comprehensive Assessments: Conduct initial interviews and thorough assessments for new residents.
- Care Planning: Develop detailed care plans focusing on short-term and long-term goals, tailored to each resident's needs.
- Collaborative Leadership: Facilitate meetings with multidisciplinary teams, caregivers, and families to align on care objectives.
- Community Collaboration: Partner with community specialists to achieve the best outcomes for residents.
- Team Integration: Participate in nursing management meetings, ensuring effective communication and collaboration at all levels.
- Education and Training: Lead in-service education sessions on MDS, RAPS, and Triggers, promoting knowledge and best practices.
- Compliance: Adhere to the MDS/Care Planning schedule, particularly for non-Medicare residents.
- Florida LPN License: Must hold a valid LPN license in the state of Florida.
- Experience: At least 2 years of experience in a Skilled Nursing Facility (SNF).
- Expertise: Strong knowledge of MDS, Medicare, and the care-planning process.
