Key Responsibilities:
- Promote Quality Care: Ensure our residents receive the highest level of care by maintaining and improving clinical standards.
- Conduct Resident Assessments: Perform comprehensive MDS assessments and interviews to create individualized, goal-driven care plans.
- Develop Strategic Care Plans: Design care strategies that promote both short-term recovery and long-term wellness.
- Foster Team Collaboration: Lead care planning meetings with healthcare providers, residents’ families, and caregivers to ensure a cohesive approach.
- Support Positive Outcomes: Partner with specialists and internal teams to enhance resident health and satisfaction.
- Lead & Educate: Guide staff through in-service training sessions on MDS, RAPS, and Triggers, and chair nursing management meetings to keep the team informed and aligned.
- Ensure Regulatory Compliance: Maintain accurate MDS schedules and documentation for non-Medicare residents.
- Current Registered Nurse (RN) license in the state of Florida
- At least 3 years of experience in a Skilled Nursing Facility (SNF)
- In-depth knowledge of MDS, Medicare, and the care planning process
