Job Overview
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in a variety of settings, including medical offices and acute care facilities. The LPN will work closely with registered nurses and physicians to ensure the best outcomes for patients, utilizing their expertise in medication administration, infection control, and discharge planning.

13 week contract: At least 12 Months Home Health experience within the last 3 years. HomeCare HomeBase is required. M-F 8am-5pm.

Duties

• Administer medications and monitor patient responses to treatments.
• Assist with discharge planning to ensure smooth transitions for patients.
• Perform triage assessments to prioritize patient care needs.
• Conduct IV insertion and manage intravenous therapy as required.
• Maintain accurate medical records and documentation using medical terminology.
• Provide care for patients undergoing dialysis and monitor their health status.
• Utilize knowledge of dermal fillers when assisting in cosmetic procedures, if applicable.
• Implement infection control protocols to ensure patient safety.
• Collaborate with healthcare team members to develop and implement individualized care plans.

Experience
Candidates should possess the following skills and experience:

• Previous experience in a medical office or acute care setting is preferred.
• Proficiency in medication administration and understanding of medical terminology.
• Experience with IV insertion techniques is highly desirable.
• Familiarity with triage processes and discharge planning is an asset.
• Knowledge of infection control practices is essential.
