Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this role, you will be responsible for coordinating patient care, ensuring that individuals receive comprehensive and effective treatment throughout their healthcare journey. You will work closely with patients, families, and other healthcare professionals to develop personalized care plans that address the unique needs of each patient. Your expertise in various nursing specialties will be invaluable in providing high-quality care and support.
13 week contract: Must have at least 12 months of hospice in home settings experience within the past 3 years. HomeCareHomeBase and OASIS experience preferred.Monday - Friday, 40 hours per week.
Responsibilities
- Assess patient needs and develop individualized care plans based on clinical evaluations.
- Coordinate with interdisciplinary teams to facilitate effective communication and collaboration in patient care.
- Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
- Provide education and resources to patients and families regarding treatment options, discharge planning, and follow-up care.
- Advocate for patients' needs within the healthcare system to ensure they receive appropriate services.
- Document all patient interactions, assessments, and care plans accurately in electronic health records.
- Participate in quality improvement initiatives to enhance patient care processes.
- Stay current with best practices in nursing and case management through ongoing education and professional development.
Experience
- A valid Registered Nurse (RN) license is required.
- Previous experience in case management or a related field is preferred.
- Familiarity with pediatrics, primary care, medical-surgical nursing, infectious disease care, inpatient care, behavioral health, PICU (Pediatric Intensive Care Unit), managed care, and discharge planning is highly desirable.
- Strong communication skills and the ability to work collaboratively within a multidisciplinary team are essential.
- Proficiency in electronic health record systems and documentation practices is necessary.
