Overview
We are seeking a compassionate and dedicated Hospice Nurse to join our team. The ideal candidate will provide high-quality nursing care to patients in a hospice setting, ensuring comfort and dignity during end-of-life care. The Hospice Nurse will collaborate with interdisciplinary teams to develop and implement individualized care plans for patients and their families.
13 week contract
Must have at least 12 months hospice experience within the last 3 years
HCHB is required
Monday-Friday 8 AM- 5 PM
$2,000-$2,100/Wk plus .42 a mile for patient related travel
Duties
- Conduct comprehensive patient assessments to determine care needs and preferences.
- Administer medications and treatments as prescribed.
- Monitor patient conditions and report any changes to the healthcare team promptly.
- Provide education and support to patients and families regarding end-of-life care options.
- Collaborate with other healthcare professionals in an ambulatory surgery center or outpatient settings as needed.
- Assist in managing symptoms related to illness, including pain management and emotional support.
Requirements
- Previous experience in hospice or palliative care is required.
- Proficiency in using HCHB is required.
- Experience in outpatient settings or ambulatory surgery centers is a plus.
