Job Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that patients receive the necessary services and support throughout their treatment journey. 13 week contract- must have at least 12 months hospice experience within the last 3 years
HCHB is required
Monday- Friday 8:30 AM- 5 PM
$2,100-$2,200/wk plus .42 a mile for patient related travel
Some On-Call will be required
Duties
- Conduct comprehensive patient assessments to identify needs and develop individualized care plans.
- Collaborate with healthcare providers to facilitate effective discharge planning and transitions of care.
- Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
- Provide education and support to patients and families regarding treatment options and resources.
- Advocate for patients’ needs within the healthcare system, including behavioral health considerations.
- Maintain accurate documentation of patient interactions, assessments, and care plans in compliance with regulatory standards.
Skills
- At least 12 months hospice experience within the last 3 years is required
- HCHB is required
- Ability to provide high-quality patient care with a focus on service excellence.
- Familiarity with emergency medicine protocols and procedures.
- Understanding of behavioral health issues and their impact on overall patient wellness.
- Excellent interpersonal skills for effective communication with patients, families, and multidisciplinary teams.
