Overview
As a Hospice Nurse, you will play a vital role in providing compassionate care to patients with terminal illnesses and their families. Your primary focus will be on enhancing the quality of life for patients through effective pain management, emotional support, and comprehensive medical care. You will work closely with an interdisciplinary team to ensure that each patient receives personalized attention tailored to their unique needs.
13 week contract
Must have at least 12 months hospice experience within the last 3 years
HCHB experience is Required
10 AM to 7 PM (or 11 AM to 8 PM) Monday through Friday.
$2,000/Wk plus .42 a mile for patient related travel

Duties
- Conduct thorough physical examinations to assess patient conditions and needs.
- Administer medications as prescribed and monitor for side effects or complications.
- Develop and implement individualized care plans, including discharge planning.
- Provide pain management strategies and interventions to enhance patient comfort.
- Maintain accurate medical records and documentation in compliance with healthcare regulations.
- Collaborate with physicians, social workers, and other healthcare professionals to coordinate care.
- Educate patients and their families about disease processes, treatment options, and end-of-life care.
- Provide support for infectious disease care as necessary.
- Utilize Epic software for efficient patient management and record-keeping.
Skills
- Proficient in pain management techniques and strategies.
- Strong knowledge of physiology and medical terminology relevant to hospice care.
- Experience in discharge planning to facilitate smooth transitions from hospital to home or hospice settings.
- Competent in medication administration practices ensuring safety and efficacy.
- Ability to perform physical examinations effectively to monitor patient health status.
- Familiarity with infectious disease protocols and care requirements.
- Experience with maintaining comprehensive medical records that meet regulatory standards.
- Knowledge of toddler care may be beneficial when working with families that include young children.
