Explorations Partial Hospitalization Program (PHP) is hiring for a full-time Case Manager. The primary objectives of the Case Manager include:
- Coordination of Care: Facilitate communication between patients, families, and treatment teams to ensure that all aspects of care are addressed.
- Resource Management: Oversee the allocation of resources within the program to maximize efficiency and effectiveness.
- Compliance and Quality Assurance: In collaboration with the Program Administrator, ensure that the program meets all regulatory requirements and maintains high standards of care.
- Patient Advocacy: Support patients in navigating the healthcare system and accessing necessary services.
DUTIES AND RESPONSIBILITIES:
Patient Intake and Assessment:
- Work in collaboration with the Lead Clinician to ensure all intake paperwork is completed.
- Collaborate with clinical staff to develop individualized treatment plans.
- Care Coordination:
- Schedule and coordinate with any collaborating agencies, and follow-up care.
- Maintain ongoing communication with patient, their families, and LEA/home school district regarding treatment progress and needs.
- Help coordinate discharge resources.
- Documentation and Reporting:
- Ensure accurate and timely documentation of patient records and case notes.
- Prepare reports for management on patient outcomes, program efficiency, and areas for improvement.
- Community Outreach:
- Develop partnerships with community organizations to facilitate resource sharing and support for patients.
- Engage in outreach efforts to raise awareness of the program and its services.
QUALIFICATIONS:
- Bachelor’s degree in Social Work, Psychology, Public Health, or a related field
- Minimum of 1 years of experience in case management or administrative roles within healthcare settings, preferably in mental health or substance abuse.
- Strong organizational and multitasking abilities.
- Excellent communication and interpersonal skills.
- Proficiency in electronic health record (EHR) systems and other relevant software.
- Knowledge of mental health regulations and compliance standards.
- TB Test/Physical (dated within one year)
- Sexual Misconduct/ Abuse Disclosure Release (Act 168)
- Official Transcripts/Diploma
- Original certificates of clearance from the State Police, Child Abuse Registry, and DHS FBI (dated within one year)
WORK SCHEDULE:
- Full-Time, Monday-Friday, 8:00am-3:30pm
SALARY & BENEFITS:
- $35,000-$40,000/year, based on education and experience.
- Bonus: $1,000 new hire signing bonus* (*after completing 6 months of service)

- Family-friendly schedule, including no nights, weekends, or holidays. Friday's off during the summer, Holiday break (one week off during December), Spring Break, and Summer break (one week off in June)

ABOUT US:
Explorations Partial Hospitalization Program (PHP) is a sub-acute program for children and adolescents with chronic or acute mental disorders who require active treatment. The program strives to meet the students’ individual and emotional needs while maintaining them within the home and school communities. The community-based program operates with a maximum of 35 students. The program focuses on stabilizing the student’s emotions and teaching the students acceptable behavior in order to transition back to the least restrictive environment with appropriate services in place.
