Case Management Specialist

Hello!

I am looking to hire a Case Management Specialist. The purpose of the Case Management Supervisor role is to provide clinical leadership to the housing support team, oversee all homeless programs as assigned, ensure accurate reporting of services to the HMIS system, and ensure excellence in homeless programs.

Responsibilities:

• Maintains a positive relationship with community partners, funding sources, owned properties, property managers, and other supportive service agencies
• Plans, implements, and supports supportive services program operation
• Make final decisions regarding admission, continuation, and termination of participant services as appropriate
• Works directly with participants who have mental health-related issues, and substance use issues, and may work with participants who have a criminal background
• Supports participants in their own homes

Qualification:

• 2-4 years of collective experience in the mental health field, substance use disorder field, and/or homeless arena (required)
• 2 years of supervisory experience (preferred)
