What are the responsibilities and job description for the Social Services Assistant position at Extendicare Health and Rehab?
Summary of Duties
Assist with the operation of the Social Service department in accordance with regulatory guidelines and Organizational established policies and procedures. Assist the Admissions team with inquiry, intake, evaluation, and admission of new residents.
Essential Job Functions
- Interview residents or family members to obtain psychosocial information.
- Complete comprehensive assessments of the psychosocial needs of each resident.
- Arrange services/appointments for residents as needed, including transportation, optometry, and dental consults.
- Review and revise care plans and assessments as necessary, but at least quarterly.
- Review complaints and grievances made by the resident and/or family and make a written/oral report to the Director of Social Services indicating what action(s) were taken to resolve the complaint or grievance following established procedures.
- Chart informative and descriptive social services progress notes of the services provided and of resident’s response to the service.
- Place orders for equipment and supplies as necessary or as may be required.
- Assist the Director of Social Services in the completion of MDS, RAPs and Care Plans.
- Assist the Director of Social Services in the development and implementation of discharge plans in conjunction with the resident, family, and other disciplines.
- Assist the Admissions team with inquiry, intake, evaluation, and admission of new residents.
- Assist in the planning and implementation of facility and community events as directed.
- Perform other duties as assigned.
Education and Experience
High School Diploma required. Two (2) years experience in long-term health care preferred.
Physical Requirements for Essential Job Functions
- Must be able to walk, sit, and stand intermittently throughout the workday.
- Must be able to reach, bend, and/or stoop intermittently throughout the work day.
- Must be able to work with hands and fingers throughout the workday.
- Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
