What are the responsibilities and job description for the Speech Language Pathologist NueRoads position at External Opportunities?
This is an Independent Contractor part-time position and pay will be based on experience.
- The Speech Language Pathologist Plans, organizes and delivers speech therapy programs that help patients/clients recover and improve their communication, language and executive processing skill, as well as meal time safety.
- Conducts examinations, evaluations, and interventions patients/clients who are affected by injury or disease.
- May treat individuals of all ages with varying diagnosis and disorders. D Will demonstrate documentation in accordance with departmental guidelines and meet all regulatory requirements.
- Demonstrates level of treatment associated with their specialty provided to their patients/clients.
- Follows and adheres to accepted clinical practice guidelines of professional organization.
- Acts as Interdisciplinary Team Member for all patient/clients receiving services.
- Works in conjunction with the Interdisciplinary Team and Personal Care Attendants, and Home Health Aides, to coordinate services to patient/clients receiving services through ARCA. Acts as ARCA representative and advocate for patient/clients receiving services and is the main liaison between the organization and the patient/clients' families and/or guardians.
- Act as an agent for the patient in regard to confidentiality. Takes detailed notes on patient/client history and progress.
- Is responsible for maintaining program eligibility for patient/clients on their caseload.
- Informs the physician and other personnel of changes in the patient/client’s condition and needs. Provide therapeutic treatments and evaluation of equipment needs as applicable.
- Establishes plan of treatment and goals.
- Developing treatment plans using a variety of treatment techniques.
- Creating personalized fitness-oriented health care programs for patients.
- Administering medically prescribed speech therapy interventions to maximize functional cognition, communication, and meal time safety.
- Advising patients on exercise techniques.
- Confer with physicians, patients and their families, nurses, therapists, social workers, and other members of a patient's care team.
- Evaluate the patient/client’s home environment and evaluating patient's progress; recommending and implementing adjustments and modifications.
- Makes regular face-to-face contact with patient/clients receiving services and may provide direct service and transportation for patient/clients when necessary.
- Performs an evaluation visit and follow-up visits as needed. Recommend and teach patients how to use adaptive equipment that will help them with daily living.
- Assess patients' progress against goals and prepare reports for healthcare providers and insurance agencies.
- Participates in pre-service training as required by the organization.
- Serves on ARCA committees as assigned.
- May be required to be "on-call" during weekend and/or during non-office hours or provide support/services to the team and/or patient/clients during emergency situations.
- Will develop, evaluate and coordinate the patient/client’s plan of care on a continuing basis.
Qualifications/Performances Requirements
Knowledge, Skills and Abilities:
- Knowledge of brain injury is preferred.
- 2 years SLP experience in in-home, or rehabilitation setting preferred
- 1 -year experience working in a community health field preferred
- Computer/technology: Computer skills and experience using Internet/Intranet, Microsoft Windows, Outlook, Word, Excel, and database applications is strongly preferred.
Pay is dependent on experience. Pay from $100.00 per hour.
