JOB SUMMARY
We are seeking a dedicated and detail-oriented Community-Based Services Support Specialist to join our team. The ideal candidate will be responsible for providing exceptional support services to our clients. This role requires strong communication skills, problem-solving abilities, and a passion for helping others.

DUTIES/RESPONSIBILITIES

  • Answers phone calls and appropriately greets all individuals.
  • Provides back up to administrative assistant during their absence.
  • Maintain an appropriate filing and auditing system for all company files.
  • Coordinate with the Compliance Supervisor to ensure files remain compliant.
  • Assist in event planning and fleet management as needed.
  • Assists with transportation and supervised visitation needs.
  • At a minimum, meet weekly with client(s) to work toward completing goals and objectives set by the Case Manager, Social Worker, Clinical Specialist, client, Community-Based Services worker and any additional members of the treatment team.
  • Provide mentor services that are both therapeutic and enjoyable to client.
  • Mentor one client at a time and tailor mentoring visits to meet each client’s individual needs.
  • Maintain documentation and administer written assessments or reports as directed.
  • Supervise, through visits and phone calls, youth and/or parents to assure the quality and follow-through on the completion of goals and objectives.
  • Identify strengths and needs and develop service plans.
  • Provide constant monitoring and assistance to a number of clients at the same time, including youth with serious health, mental or physical conditions and/or disabilities.
  • Report concerning incidents or injuries immediately to a supervisor.
  • Educate and counsel the youth and family in order to achieve goals established on the service plan.
  • Stabilize foster care placements by providing services including individualized training and skill development for parents, home visits, referral to community resources and specialists, family advocacy, on-call crisis intervention, progress reports, and service planning.
  • Stabilize families at risk of their children being removed from their care by providing individualized training and skill development for biological parents/legal guardians, parenting and co-parenting skills training.
  • Attend court, school, and family partnership, team decision, intensive care coordination, FAPT, and other meetings as required.
  • Provide a positive role model, help clients develop appropriate relational skills, connect clients to needed resources, and aid clients in attaining life skills specific to their service plan needs.
  • Develop and enforce behavior management strategies implemented to address any presenting issues.
  • Individualized training and skill development which may include, but is not limited to, conflict management, child development education, developmental and behavioral effects of trauma education, domestic violence education, parenting education and training, parent training with children present, monitoring of progress of parenting skills, family communication, development of structure and time management, Reactive Attachment Disorder (RAD) support, behavior modification, budgeting and money management, meal planning and preparation, and development of long and short-term goals.
  • Assist with crisis management.
  • Assist with the development of psycho-educational curriculum and facilitation of activities during monthly foster parent training.
  • Participate in ongoing supervision and staff development.
  • Safe and timely transportation of clients to and from school and other appointments as needed.
  • Abides by the transportation minimum driver guidelines of Extra Special Parents.
  • Ensures safety of children through the proper use of car seats and seat belts.
  • Attends program meetings, including but not limited to, weekly team meeting, community-based team meeting, and any others as required.
  • Reports any ethical and legal violations to supervisor.
  • Performs other related duties as assigned.

KNOWLEDGE/SKILLS REQUIRED

  • Knowledge of the Therapeutic Community model.
  • Knowledge of Trauma Informed Care.
  • Ability to maintain appropriate confidentiality and comply with all HIPAA requirements.
  • Strong verbal and written communication skills.
  • Ability to effectively and accurately draft varying correspondence.
  • Ability to establish and maintain accurate records and files.
  • Strong customer service and problem-solving skills.
  • Knowledge of transportation and child safety seat laws.
  • Strong time management and organizational skills.
  • Ability to complete pre-service and annual in-service CPR and first aid certification training.

EDUCATION / EXPERIENCE

  • A baccalaureate degree in social work from a college or university accredited by the Council on Social Work Education or a field related to social work such as sociology, psychology, special education, or counseling, plus one year of experience providing services to children and their families is preferred; OR
  • A baccalaureate degree in any field plus two years of experience providing services to children and their families; OR
  • A high school diploma or equivalent plus 5 years of experience providing services to children and their families.

OTHER REQUIREMENTS

  • Must be at least 21 years of age or older.
  • Candidate is required to possess a valid VA state driver's license as it is necessary to perform the essential duties of the position.
  • This is a Part-time, hourly position; evening and/or weekend hours may be necessary to meet the needs of clients.
