As a Care Coordinator, you will work closely with peer support specialists, service providers, and community organizations to create a coordinated and person-centered approach to care. Role Responsibilities:
- Act as the primary point of contact, advocate, and resource for individuals enrolled in F5 Project’s care coordination programs (e.g., Free Through Recovery, Community Connect), working alongside care teams, family members, probation/parole officers, and community partners.
- Assess and monitor participant needs, develop individualized care plans, and set client-driven goals to support recovery, housing, employment, and overall well-being.
- Conduct regular check-ins with participants, monitoring progress and adjusting care plans as necessary.
- Educate participants on available community resources and help them navigate services to support their recovery and overall well-being.
- Facilitate communication between participants and service providers to ensure seamless care coordination and access to necessary resources
- Facilitate access to inpatient treatment centers for select participants
- Complete all required Care Coordination training, ongoing professional development, and actively participate in Care Coordinator meetings.
- Ensure strict compliance with HIPAA guidelines by maintaining participant confidentiality at all times.
- Maintain accurate records and documentation while staying organized in a fast-paced environment.
- Foster strong community partnerships by collaborating with local agencies and service providers
- Demonstrate professionalism and strong communication skills, and ethical care coordination practices in all interactions.
- Travel up to 20% of the time, as needed
- Perform other duties as assigned
Required Qualifications:
· Requires 3 years of experience with special populations, or a bachelor’s degree and 1 year of supervised experience
· Must be passionate about providing direct services and building rapport with individuals, especially those from special populations
· Must complete state-mandated care coordination training upon hire
· Hold a valid driver’s license and be able to pass a background check
· Candidate must reside in Region 4: Grand Forks, Pembina, Nelson, or Walsh County
Preferred Qualifications:
· Work experience in case management, behavioral health services, or social work
· Licensed Bachelor of Social Work (LBSW) or actively working towards this credential
