About the Job
Fair Haven Community Health Care is a forward-thinking, dynamic, and exciting community health center providing care for multiple generations at over 143,000 office visits in 21 locations. We are seeking a Community Care Navigator to join our team and help us provide excellent, affordable primary care to all patients, regardless of insurance status or ability to pay.
In this role, you will work closely with our clinical teams to identify and address the social determinants of health that impact our patients' ability to adhere to treatment plans. You will also develop and evaluate shared plans of care, link patients with social needs to community resources, and conduct home visits as needed.
Requirements
- A bachelor's degree in a health-related field or equivalent.
- A valid CT driver's license and/or access to reliable transportation.
- Experience in care coordination, working with teams, and using an electronic health record.
- Bilingual Spanish/English is required.
