About the Role
As a Patient Care Navigator for Harm Reduction, you will play a vital part in our team at Fair Haven Community Health Care. Your primary responsibility will be to coordinate client intakes and non-clinical services at our shelter- and street-based facilities. You will work closely with clients to identify their unique needs and provide them with the necessary support to access various community resources.
Duties and Responsibilities
- Liaison with external organizations and care providers to link clients to suitable services
- Develop and evaluate shared plans of care tailored to individual needs
- Link patients with social needs to relevant community resources
- Assist with and follow up on health maintenance items and chronic disease management
- Conduct home visits as required
- Identify barriers to care impacting clients' adherence to treatments
- Collaborate with clinical teams to meet the clinical and social needs of high-needs patients
- Attend relevant trainings and maintain strict adherence to deadlines and documentation
- Coordinate essential services such as food, laundry, and shower for clients at shelter-based facilities
Qualifications and Skills
To be successful in this role, you must possess a Bachelor's degree in a health-related field and a valid driver's license or reliable transportation. Experience in care coordination, working with teams, and using electronic health records is highly desirable. Bilingual Spanish/English proficiency is also required.
