Your Key Duties
- Provide patient navigation and care coordination services to ensure seamless access to care
- Liaise with external organizations and care providers to link clients to available services
- Develop and evaluate shared plans of care tailored to individual needs
- Link patients with social needs to relevant community resources
- Assist with and follow up on health maintenance items and chronic disease management
- Conduct home visits as required
- Identify barriers to care impacting clients' adherence to treatments
- Collaborate with clinical teams to meet the clinical and social needs of high-needs patients
- Document referrals, encounters, and services in the EPIC electronic health record
- Maintain adherence to deadlines and documentation requirements
- Coordinate essential services such as food, laundry, and shower for clients at shelter-based facilities
