Your Key Responsibilities
- Screen for social determinants of health using validated screening instruments on all clients at least yearly
- Liaise with other organizations and care providers to link clients to available services
- Develop and evaluate shared plans of care to address clients' unique needs
- Link patients with social needs to community resources
- Assist with and follow up on health maintenance items and chronic disease management
- Conduct home visits as needed
- Identify barriers to care impacting clients' abilities to adhere to treatments
- Work collaboratively with clinical teams to meet the clinical and social needs of high-needs patients
- Document referrals, encounters, and services in the EPIC electronic health record
- Maintain adherence to deadlines and documentation requirements
- Coordinate essential services such as food, laundry, and shower for clients at shelter-based facilities
