GRANT SUMMARY
The goal of the Athabascan Behavioral Health Clinic project is to transform the community behavioral health system by providing comprehensive, coordinated behavioral health care. The Clinic provides culturally specific programming, strength-based services, community-wide engagement, evidence-based practices, and early intervention.
POSITION SUMMARY:
This position is responsible for providing case management services for project participants (consumers). The position assists consumers in learning coping skills and creating stability in their lives. This position is part of a collaborative treatment team and serves as a liaison between consumer and clinical staff assuring culturally resonant intervention, treatment, and support services.
JOB DUTIES
- Provides targeted case management services assisting individuals in sustaining recovery for ten (10) consumers.
- Develops individualized Case Management Plans that includes access to care, cultural competency guidance, trauma-informed care, workforce development & training, and consumer-paced sustainability.
- Assesses System of Care (SOC) collaboration with the Behavioral Health Community Coalition (BHCC) and coordinates activities and evaluates outcome annually.
- Makes appointments for allied and support care.
- Mentors consumers through referral intake documentation. Follows-up on referral care.
- Advocates for consumers.
- Refers consumers to SBIRT and conducts intake to Case Management services.
- Works with consumers to understand and address risk factors and connects consumers with resources and support services.
- Assists with Medicaid and health insurance eligibility, linkage to primary and dental care, and linkage to community organizations through the BHCC.
- Participates in continuous quality improvement and data-driven decision-making processes to assess program efficacy and drive quality improvement.
- Ensure adherence to data collection requirements and enters accurate and timely data into the Electronic Health Record (EHR).
- Provides culturally relevant and trauma informed service.
- Attends mandatory position training that provides training in risk factors.
- Performs other job-related duties as assigned.
NECESSARY KNOWLEDGE, SKILLS, AND ABILITY
- Ability to report to work in a timely manner.
- Knowledge of substance abuse and mental health; and appropriate skills in working with consumers with Mental Health/Substance Use disorders.
- Skill in working successfully with the Alaska Native/American Indian population and experience providing culturally specific programming; understanding of and sensitivity to diverse cultures and lifestyles.
- Skill in operating/using computers and using Windows, Word, and Excel with ability to accurately input information into reporting formats.
- Above average communication and people skills with the ability to accurately follow written and verbal instructions.
- Knowledge on how to best assist possible escalated individuals during data collection process.
- Ability to protect confidentiality is essential; HIPAA & 42 CFR compliant.
- Knowledge and ability to interpret/understand billing procedures.
- Skill in managing multiple priorities and tasks concurrently and meeting their deadlines.
- Skill in establishing and maintaining cooperative working relationships with colleagues, supervisors, agencies, and consumers.
- Ability to work independently as well as with teams.
MINIMUM QUALIFICATIONS (Education & Experience)
- Bachelor’s degree in behavioral health or related field.
- Two (2) years’ experience working with AN/AI population and experience providing culturally specific services.
- Two (2) years’ experience involving case management.
- A combination of experience, education, and training that provides the required knowledge, skills, and abilities may substitute for degree requirements.
PREFERRED QUALIFICATIONS (Education & Experience)
- Experience working with electronic health records.
- Master’s degree in behavioral health or related field.
