NATURE OF POSITION
The Care Coordinator assists with patient care coordination of for both Utilization and Disease Management/Wellness to produce quality patient outcomes. Under the direction and supervision of the Director of Community Outreach. The Care Coordinator assists patients by providing support with treatment plan compliance; communication and education; and by assisting in the collaborative coordination of care to reduce waste and inefficiency while improving patient quality outcomes
QUALIFICATIONS
- High school graduate. Some college education is preferred in the health professional field of study.
- Healthcare-related experience is preferred.
- Strong organizational and time management skills, able to work with little supervision while handling numerous projects at once.
- Excellent written/verbal communication skills, especially the ability to communicate telephonically in a professional and effective manner.
- Computer literacy in Word and Excel.
SALARY GRADE and CLASSIFICATION: Non-Exempt
RESPONSIBLE TO: Director of Community Outreach
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provides telephonic follow up with patients for Care Management services to include post hospital discharge and chronic condition/wellness management
- Complete assigned tasks related to care coordination
- Working with patient, physician and/or other care providers to meet the patient’s individual needs based on protocols and referrals producing improvement in quality outcomes
- Calling the identified patient to offer a review of the Disease/Wellness Management program
- Supporting the patient education goals regarding their specific chronic condition based on the plan of care
- Informing the patient how to access resources
- Assisting in the arrangement of Provider follow-up appointments and delivery of care
- Mailing of approved educational and informational material
- Assisting in the performance of calls to patients post discharge from inpatient or other levels of care
- Assist in special events planned and organized by the Outreach department as applicable
- Supporting the monitoring and evalauation of interventions int he plan of care
- All communications will be conducted in a professional manner
- Administers tasks as detailed in the patient specific plan of care
- Completes clear and concise documentation in EMR
- Participates in health plan reviews
- Assists with handling incoming and outgoing calls: answering questions regarding the referrals and obtaining information as requested by licensed staff
- Assists with data entry, faxing, filing and other clerical tasks
- Complies with customer service expectations as applicable to the department and health plan
- Comply with all Fairfax Medical Facilities, Inc. policies and procedures
- Participate in departmental and company in-services as appropriate
- Recognizes and adheres to the scope / limitations of job requirements
- Generate tasks to the assigned Case Manager-RN or clinic medical staff as applicable
- Performs other related duties as assigned
