REGULAR HOURS OF WORK: 8:00 a.m. - 4:30 p.m., 37.5 paid hours per week; (hours may be flexible, not to exceed 40 hours per week)

SPECIFICATIONS:

As an employee of the Fairfield County Board of Developmental Disabilities, (FCBDD) the job incumbent shall comply with all Board policies at all times, and shall demonstrate respect for, support dignity of, and observe the rights of all individuals served by the agency.  Job incumbent will clarify to all stakeholders that the Fairfield DD vision is to support people on their journey toward independence.  

45%Implements the Major Unusual Incident and Unusual Incident Processes for the FCBDD Demonstrated by:
  • Reviews all Unusual Incident Reports (UIs) received to determine if an MUI has occurred, which may involve screening, and consultation with Supervisor, DODD, or others as needed.
  • Ensures response /follow-up to incidents received if necessary; by contacting internal or external providers for additional information or to make recommendations or compel others for action. These communications with internal providers may include Directors or other management employees.
  • Uses Unusual Incident database; training others to enter incidents, and periodic review of quality of data entered with supervisor. Outcomes of reviews may necessitate making recommendations and/or compelling others to enter additional information relative to documentation of clarification and follow up. 
  • Provides accessibility to internal and external providers to consult with how to approach incidents, and/or performs crisis intervention.  Accessibility is attained through personal contact, phone, email, and remote access if needed.  Recommendations on how to proceed with incidents may be requested, or once a situation is identified, the need to contact the necessary parties to address the issue may be necessary.  
  • Assures compliance to DODD regulations regarding UIs/MUIs. 
  • Consults with contracted investigative agent agency to determine if an incident rises to the current interpretation of a reportable MUI.
  • Reviews initial MUI reports received and submits to the contracted investigative agent agency so they can complete the initial filing through ITS within required timelines and procedures and begin the investigation.  
  • Recommends to or compelling others to provide immediate actions or a prevention plan relative a MUI through communication with internal and external customers, including recommending revisions to Individual Service Plans to teams.  Ensures prevention plans align with the FCBDD mission.
  • Completes prevention plan review within 30 days after plan submission to ensure that the plan has been implemented and is effective. 
  • Maintains MUI files and required documentation, including the collection of needed information, ensuring it is forwarded to the Investigative Agent or other investigative entity [law enforcement].  
  • Maintains of MUI database that is stored on network server. 
  • Provides written facts and findings to all required parties relative to the outcome of MUI investigations.
  • Assures that all aspects of assigned MUI investigations are properly completed and closed within established timelines.
  • Acts as a liaison between all parties including individuals, providers, families, public officials, etc.; this may be in the function of UI incident review, or consultation with other agencies navigating through the DD system relative to an incident involving an individual.
  • Maintains discreet confidentiality of high-profile incidents or situations involving internal or external providers which often involves human resources information that may result in a personnel disciplinary action. 
  • Reviews a representative sample of provider unusual incident logs each month to ensure that MUIs have been reported, preventive measures have been implemented, and that trends and patterns have been identified and addressed. 
  • Participates in annual reviews of MUIs by a committee composed of a cross section of individuals representing county board staff, private providers, families, and other stakeholders.  
  • Prepares/participates in any reviews conducted by the DODD and other regulatory/accrediting agencies including preparation of any outcome plan.
  • Performs other duties as necessary to assure compliance to agency policies and procedures or DODD regulations related to Major Unusual Incidents and Unusual Incidents.
15% Court Liaison
Develops and maintains effective communications with local law enforcement and other agencies to enhance a collaborative approach in all situations. 
Maintains discreet communications relative to high profile cases that may involve law enforcement, prosecution or other responsible entity.  
Consults as needed in situations where individuals with disabilities that may or may not be receiving services from Fairfield DD.
Connects law enforcement, prosecution or other responsible entity with Individual Support Coordinator and other staff at the earliest opportunity to provide people in the legal system the greatest level of support from Fairfield DD.
 
20%Communications
Interacts in a courteous and professional manner with individuals receiving supports, their families, and friends and with coworkers, supervisors and members of the community. Actively promotes good public relations with parents, advocates, community organization, professionals, other school districts, Job and Family Services, other agencies and other county board staff.
Communicates with Individual Support Coordinators, Personal Advocates, Behavior Coordinator, community employment, and other staff as well as home providers, parents, guardians, and other agencies as needed.
Develops and maintains effective communications with local law enforcement and other agencies to enhance a collaborative approach towards any situation.
Remains calm in highly emotionally charged situations, provides concise, accurate information, and conveys respect, and support to individuals, families, internal and external providers and outside entities.  

15%   Training
Assists supervisor in providing presentations and technical assistance concerning DODD regulations and county board policies/procedures to county board staff and external providers as required per regulations.
Provides training and technical assistance to providers on best practices concerning annual MUI training, writing incident reports, completing incident logs, annual MUI reviews, etc.
Offers relevant training as part of the prevention planning process on a case-by-case basis. 
 
5% Other Miscellaneous Duties
Maintains up-to-date knowledge of regulations, trends, and issues that are related to the satisfactory completion of job.
Assesses providers’ compliance to all applicable regulations and contract terms for the provision of services/supports as identified in an individual's plan due to the result of a Major Unusual Incident or trends and patterns from a review of Unusual Incidents.
Monitors the implementation of systems changes as identified through review of or trends and patterns identified from Major Unusual Incidents or Unusual Incidents. 
Assists in the identification, assessment and development of staff training needs in support of the implementation of services related to MUI/UI trends.
Completes documentation for Medication Administration Claiming or other designated funding source regulations and agency procedures.
Participates in community outreach committees.  These committees may request information relative to the services and supports that FCBDD provides to individuals.
Complies with the Safety Rules established for the purpose of fulfilling compliance with Ohio Employment Risk Reduction Standards, Rules, or Orders, through the utilization of protective equipment and decontamination techniques. 
Performs other duties as directed.QUALIFICATIONS:
  • Education:
    • Bachelor’s degree or higher in psychology, education, social work, or other related field that contributes to the employee's ability to perform job functions; OR
    • Associate’s degree or at least junior year standing and two years full-time (or PTE) supervised, paid work experience in conducting administrative, civil, and/or criminal investigations; OR
    • Associate's degree or at least junior year standing and two years full-time (or PTE), supervised, paid work experience in administration, supervision, instruction, or habilitation of individuals with developmental disabilities; OR
  • Maintain DODD, First Aid and CPR certifications and current behavior support training.
  • Minimum 2 years’ experience as an employee in a county board or other provider where applicant has knowledge of support systems available to individuals.
  • Must have a valid Ohio driver's license, points on license may not be equal to or exceed 6 and be insurable under agency insurance carrier. 
  • May be requested to work abnormal hours based on service needs.
  • May use personal vehicles to transport clients and families.
  • Maintain certification to conduct DODD protocol Provider Compliance Reviews through the RDS system.
  • Maintain training related to MUI/DODD ITMS. 
  • Communicate professionally and effectively with persons served, parents, providers, guardians and agencies.
  • Ability to write clearly and concisely.
  • Ability to maintain accurate records and data.
  • Ability to cope with stressful situations. 
  • Ability to pass all background checks including Bureau of Criminal Identification and Investigation (BCII).
  • Must be physically capable to lift up to 50 lbs. per lifting policy, move enrollees, including children, adolescents and adults in a safe manner, according to in-service training.
KNOWLEDGE, SKILLS AND ABILITIES:
  • Ability to work independently and plan own work.
  • Ability to work with flexibility and ability to reprioritize as needed. 
  • Knowledgeable of available community resources to individuals eligible for county board services
  • Works cooperatively with others.
  • Maintains attendance to effectively complete functions of the job.
  • Communicates both orally and written and presents information in an objective, professional manner.
  • Understands the philosophy of self-determination/person centered planning.
  • Ability to interact with families, private citizens, and others in a professional manner to promote a positive image of persons with disabilities and the agency.
  • Possess computer skills to complete Medication Administration Claiming (MAC) documentation; for utilizing the DODD Incident Tracking system, and other assignments using word processing, basic spreadsheets, create presentations and databases.  Internet research skills, data analysis, and report writing is also required.  
  • Organizational, clerical and technical skills necessary to maintain records on the computer server.
  • Ability to recognize unusual or threatening conditions and take appropriate action.
  • Ability to work under stress and time constraints.
  • Uses good judgment to define problems, collect data, establish facts, and draw valid conclusions.
  • Maintains strict adherence to the confidentiality policy and procedures related to individuals and personnel.

PROBATIONARY PERIOD: One (1) calendar year


STATUS: Classified Civil Service, hourly, non-exempt from overtime
