We are seeking a detail-oriented Nurse to join our team as our MDS Coordinator. This is a salaried (exempt) position with a base Monday to Friday schedule.
Compensation and Schedule:
- Salary Range: $65,000 - 85,000 (depending on experience)
- Schedule: Salaried
Note: The MDS Coordinator participates in the on-call rotation
Essential Functions: Oversees scheduling of the MDS assessments to maintain federal and state regulatory compliance. Coordinates all functions of the MDS assessment and care planning process. Participates in and/or coordinates interdisciplinary meetings. Participates in determining resident level of care. Conducts care plan conferences. Participates in tracking resident events such as falls, antibiotic use, psychotropic drug use and significant changes. Is an essential part of the Nursing Leadership team.
Qualifications:
- Must be a graduate from an accredited school of nursing with an active Illinois R.N. or L.P.N. license.
- Must be able to read and follow regulations and instructions,
- Able to use the computer for a major portion of job duties.
- Must be accurate and detail-oriented.
- Knowledge of geriatric population within a life community setting helpful.
- Two years' experience preferred.
